1. Ensure Patient Safety First

3. Verify Both Vaporizers Are Turned Off

If the known-good vaporizer operates normally, remove the original vaporizer from service for evaluation.
If the known-good vaporizer produces the same fault, the mounting position, interlock mechanism, or anesthesia machine requires bench evaluation.
Do not use a vaporizer containing the wrong anesthetic agent merely for convenience. Follow facility controls for agent-specific vaporizers.
Expected outcome: The fault is isolated to either the vaporizer or the anesthesia machine.
10. Restart and Complete the Machine Checkout
If the configuration includes vaporizer identification or related electronic monitoring:
- Turn all vaporizers OFF.
- Confirm each vaporizer is properly seated and locked.
- Restart the anesthesia machine.
- Allow startup to complete without manipulating the vaporizer controls.
- Review displayed vaporizer information and alarms.
- Perform the complete manufacturer-required pre-use checkout.
- Verify anesthetic-agent output with an approved gas analyzer when required by facility procedure.
Expected outcome: The machine completes checkout without vaporizer or interlock faults, and measured agent concentration responds appropriately.
If the problem resolves and all required testing passes, return the device to service according to facility policy and stop troubleshooting.
If the Problem Persists
If compatible vaporizers are correctly installed, both controls return fully to OFF, mounting surfaces are clean, and the interlock still fails, common external causes have been ruled out.
The problem may involve the vaporizer’s locking mechanism, manifold interface, interlock assembly, position-sensing components, or internal control circuitry.
The anesthesia machine should be:
- Removed from service
- Labeled Out of Service
- Sent for qualified repair or bench evaluation
Any vaporizer that cannot be securely mounted, cannot be reliably switched off, leaks, delivers questionable output, or permits simultaneous vaporizer activation must also be removed from service.
