1. Ensure Patient Safety First
Do not perform extended EtCO2 troubleshooting while the LIFEPAK 12 is actively monitoring a patient whose ventilation status depends on capnography.
If the capnogram disappears during patient care:
- Immediately assess the patient and verify ventilation by an alternate clinical method.
- Check the airway, breathing circuit, and ventilator for disconnection, obstruction, or leakage.
- Transfer EtCO2 monitoring to another verified monitor when continuous capnography is clinically required.
- Do not assume that a missing waveform is only an equipment problem.
Expected outcome: Patient ventilation is confirmed independently, and care does not rely on an unreliable EtCO2 channel.
Continue Clinical Engineering troubleshooting only after the device has been safely removed from patient use.
2. Confirm the Exact Failure
Power on the LIFEPAK 12 in a controlled, nonclinical area and document whether the complaint involves:
- No CO2 numeric value
- No capnogram
- CO2 FILTERLINE OFF
- CO2 FILTERLINE BLOCKAGE
- CO2 FILTERLINE PURGING
- ALARM APNEA
- XXX displayed instead of an EtCO2 value
- Erratic or consistently inaccurate readings
The EtCO2 module begins initialization when compatible CO2 tubing is connected, and initialization, self-test, and warm-up may take up to approximately two-and-one-half minutes.

If the waveform and numerics appear normally after initialization, complete a functional verification and stop.
3. Verify the EtCO2 Option Activates
Connect a compatible Microstream FilterLine securely to the LIFEPAK 12 CO2 connector.
Confirm that:
- The connector is inserted into the correct CO2 port.
- The threaded connector turns clockwise and seats securely.
- The CO2 parameter area appears on the display.
- The module begins its initialization sequence.
A loose or disconnected FilterLine may produce CO2 FILTERLINE OFF or prevent sampling from starting.
Expected outcome: The LIFEPAK 12 recognizes the sampling line and activates the EtCO2 display.
If recognition remains absent with a verified accessory, continue troubleshooting.
4. Inspect the Sampling Line and Connector
Remove the FilterLine and inspect it externally for:
- Kinks or sharp bends
- Crushed or stretched tubing
- Loose fittings
- Visible moisture, blood, secretions, or debris
- Damaged threads
- A cracked airway adapter or cannula
- Evidence that the disposable accessory was previously used
Inspect the LIFEPAK 12 CO2 inlet for visible contamination or physical damage without inserting tools or fluids into the port.
Expected outcome: The complete sampling path is open, dry, securely connected, and undamaged.

5. Replace the Disposable Sampling Assembly
Install a new, unopened, compatible FilterLine, airway adapter, or nasal sampling cannula.
Do not reuse the original line for the verification test, even when no obstruction is visible. Small amounts of moisture or internal contamination can restrict flow without being externally apparent.
Allow the EtCO2 module to complete initialization.
Expected outcome: The replacement accessory is recognized, and a stable CO2 waveform and numeric reading appear.
If this resolves the problem, discard the failed disposable accessory according to facility procedure, document the result, and stop.
6. Address FilterLine Blockage or Purging Messages
For CO2 FILTERLINE BLOCKAGE:
- Disconnect the sampling line.
- Check for kinks, moisture, secretions, or a clogged airway adapter.
- Reconnect the line once.
- Replace the entire sampling assembly if the message returns.
For CO2 FILTERLINE PURGING:
- Inspect the tubing for water or obstruction.
- Untwist or replace the line.
- Reconnect it securely.
- Allow the purge cycle to complete.
Rapid altitude or ambient-pressure changes may temporarily trigger purging. Physio-Control directs users to remove and reconnect the FilterLine when purging or blockage messages occur under these conditions.
Expected outcome: The message clears and normal sampling resumes.
If blockage or purging continues with a new line under stable environmental conditions, continue troubleshooting.
7. Check for Sampling-Path Leaks
Using an approved capnography simulator or controlled breath source, inspect the external sampling path for:
- Loose threaded connection at the monitor
- Loose airway-adapter connection
- Cracked tubing or fittings
- An improperly assembled test setup
- A leaking breathing-circuit connection
Do not use an uncontrolled clinical patient test for bench troubleshooting.
Expected outcome: The sampling path remains sealed and produces a repeatable waveform.
If reseating or replacing the accessory corrects the waveform, complete final verification and stop.
8. Verify the Displayed CO2 Waveform Channel
Confirm that the CO2 waveform is assigned to an available waveform channel and is not merely hidden by the current display configuration.
Select the CO2 parameter and verify:
- CO2 is displayed in Channel 2 or Channel 3.
- The scale is appropriate.
- Autoscale is selected when suitable.
- Display contrast is sufficient to see the waveform.
The LIFEPAK 12 provides Autoscale, 0–50 mmHg, and 0–100 mmHg CO2 scale options.
Expected outcome: The capnogram is visibly displayed at a usable scale.
If numeric EtCO2 is present but no waveform is displayed, a display-channel configuration issue may have caused the complaint.
9. Test with Known-Good Accessories and a Simulator
Connect:
- A new compatible Microstream sampling line
- A known-good airway adapter or cannula
- An approved capnography or patient simulator capable of producing a repeatable CO2 waveform
Verify:
- Sampling-line recognition
- Module initialization
- Waveform presence
- Respiratory-rate indication
- Stable EtCO2 numeric response
- Absence of blockage, purging, or service messages
Expected outcome: A stable, repeatable capnogram and corresponding EtCO2 value are displayed.
If the device passes with known-good accessories, the original sampling accessory or external setup was the likely cause.
10. Evaluate Persistent Calibration or Module Indicators
If XXX appears instead of the EtCO2 value, or readings remain consistently inaccurate with a verified simulator and new sampling line, do not attempt correction through repeated accessory changes.
The manufacturer associates XXX with unsuccessful CO2 calibration or CO2 module failure and directs the issue to qualified service personnel. It also specifies at least annual EtCO2 calibration by qualified service personnel.
Expected outcome: The technician recognizes that external causes have been ruled out and stops before unauthorized internal repair.
If the Problem Persists
If the LIFEPAK 12 will not recognize a new compatible FilterLine, repeatedly reports blockage or purging, displays XXX, or fails to produce a waveform from an approved simulator, common external causes have been ruled out.
The problem may involve the:
- CO2 inlet connector
- Internal sampling pump
- Pressure or flow detection system
