Step-by-Step Troubleshooting
1. Protect Patient Safety and Load Integrity
Do not consider an incomplete or under-temperature load sterile. Quarantine the affected load according to facility policy and maintain traceability.
Move future processing to another verified sterilizer until the temperature problem is resolved.
Expected outcome: No questionable load is released for patient use.
2. Confirm the Exact Cycle Failure
Review the available cycle record and staff report. Determine:
- Which cycle was running.
- Whether the unit heated partially or remained near initial temperature.
- When the failure occurred.
- Whether pressure increased normally.
- Whether an alarm or message appeared.
- Whether the issue affects multiple cycles.
Do not rely solely on verbal description if a cycle record is available.
Expected outcome: The temperature complaint is confirmed and correlated with the observed cycle behavior.
3. Check the Load and Cycle Selection
Confirm the selected cycle was appropriate for the load and that the chamber was not obviously overloaded or packed in a way that could interfere with steam contact or air removal.
Do not alter validated parameters to compensate for a failure.
Expected outcome: The load and cycle selection are appropriate. If an incorrect cycle or obvious loading issue explains the event, correct it and repeat only according to facility policy.
4. Verify Facility Steam Availability
Check whether the sterilizer is receiving the required facility steam service and whether other steam-using equipment is experiencing problems.
Inspect accessible isolation valves for an obvious closed condition if this is within Clinical Engineering responsibility.
Expected outcome: Steam is available to the sterilizer and no facility-wide supply problem is identified. If a facility steam interruption is found, route the issue appropriately and do not continue equipment-level troubleshooting until supply is restored.
5. Check Chamber Pressure Behavior
Review the cycle display or record to determine whether chamber pressure rises as heating is requested.
Temperature and pressure behavior should be considered together when determining whether steam is entering the chamber normally.
Expected outcome: Chamber pressure responds consistently with heating. Little or no pressure increase suggests a steam-supply or steam-admission problem requiring further evaluation.
6. Inspect for Obvious External Steam or Water Leaks
Look around accessible piping, door areas, and the sterilizer perimeter for visible steam, condensation, water accumulation, or abnormal noise.
Do not approach or touch hot leaking steam components.
Expected outcome: No obvious external leak is present. Any significant steam leak requires the sterilizer to be removed from service and the area handled according to facility safety procedures.
7. Check Drain Condition
Verify the chamber drain area is not visibly obstructed by debris and that water is not accumulating abnormally in the chamber.
A drainage problem can interfere with proper cycle conditioning and temperature performance.
Expected outcome: The chamber drain is visibly clear and there is no abnormal standing water. If an accessible obstruction is safely removed and normal heating returns, proceed to final verification.
8. Verify Door Closure and Seal Condition
Check that the chamber door closes and locks normally and that accessible sealing surfaces are clean and free of obvious damage.
A sealing problem may prevent the chamber from maintaining the conditions required for normal heating.
Expected outcome: The door is properly secured and no obvious external sealing defect is present.
9. Compare With an Approved Test Cycle
After external issues are corrected, perform an appropriate facility-approved test cycle. Observe chamber temperature and pressure progression without modifying validated settings.
Expected outcome: The sterilizer reaches and maintains the expected cycle conditions without abnormal interruption. If successful and all required performance checks pass, troubleshooting can stop.
10. Escalate Persistent Heating Failure
If steam availability, load, cycle selection, drainage, door closure, and external leak conditions are satisfactory but the sterilizer still fails to reach sterilization temperature, stop external troubleshooting.
Service-level categories may include steam-control components, temperature sensing, pressure sensing, traps, internal piping, control systems, or utility quality.
Expected outcome: The sterilizer remains out of service until evaluated and proven capable of achieving validated conditions.
If the Problem Persists
External causes have been reasonably ruled out. Remove the sterilizer from service, label it Out of Service, and arrange qualified repair or service evaluation.
Use current STERIS documentation and approved test equipment to evaluate steam control, sensing, internal plumbing, and control functions. Do not make unsupported calibration or service-menu changes.
Complete all applicable operational, calibration, and sterilization-performance testing before return to service. Knowing when to stop and escalate prevents repeated failed loads and inappropriate assumptions.

Any load from a cycle that failed to achieve required sterilization conditions must be treated as nonsterile until properly reprocessed.
