Step-by-Step PM Procedure

1. Confirm Device Identity and Condition

Verify the monitor is a Nihon Kohden BSM-6000 Series bedside monitor. Confirm model, serial number, asset tag, location, and installed options.

Expected outcome: Device identity matches the CMMS record.

2. Remove From Patient Use

Confirm the monitor is not connected to a patient. Remove patient cables, sensors, cuffs, and applied parts before PM unless needed for a controlled test.

Expected outcome: PM is performed off-patient.

3. Visual Inspection

Inspect the monitor housing, display, touchscreen, controls, AC power cord, equipotential terminal, battery condition, connectors, modules, recorder if installed, network ports, and accessory sockets.

Expected outcome: No cracked housing, damaged power cord, loose connectors, missing labels, exposed conductors, liquid intrusion, or unsafe accessory condition.

Remove from service if physical damage, liquid ingress, damaged power cord, failed grounding, failed leakage test, failed alarm indicator, failed temperature check, or any fault that may affect safe monitoring is found.

4. Power-On Check

Power the monitor on using AC power. Observe startup behavior and confirm the monitor boots normally without unresolved technical alarms or fault messages.

Expected outcome: Monitor powers on normally and reaches normal operating display.

5. Access Diagnostic Check Screen

Turn monitor power off. Press the Power switch while pressing the Silence Alarms key until the DIAGNOSTIC CHECK screen appears.

Expected outcome: Diagnostic check screen opens.

Note: The manual states this screen can be used to view error history or perform manual check, system setup, or initialization. Do not change settings during PM unless specifically required by facility procedure.

6. Access Manual Check Screen

From the Diagnostic Check screen, select MANUAL CHECK, enter the password, and open the Manual Check screen.

Expected outcome: Manual Check screen opens.

7. Alarm Indicator Check

Open the ALARM INDICATOR check from the Manual Check screen. Check that the top alarm indicator lamp lights according to the displayed screen indication. Use AUTO or manually check the listed colors.

Expected outcome: Alarm indicator lights correctly for each indicated color.

Fail condition: If a color does not light, lights partially, or does not match the screen indication, remove from service for repair.

8. Temperature Accuracy Check

Connect the AX-400G vital sign simulator to the monitor temperature input.

Use the appropriate cable: TEMP socket with AX-800P compatible cable, or MP socket with BSS cable for AX-400G.

Set the AX-400G output and confirm the monitor indication is within the manual-listed range:

Expected outcome: Displayed temperature is within the listed range at each test point.

9. Temperature Connector-Off Detection

During temperature signal input from the AX-400G, disconnect the temperature connection cord from the MULTI socket.

Expected outcome: Monitor displays CONNECTOR OFF.

10. Temperature Sensor-Off Detection

During temperature signal input from the AX-400G, remove the temperature cable plug from the TEMP socket.

Expected outcome: Monitor displays CHECK SENSOR.

11. Restore Settings

After temperature testing, return any changed temperature settings to the facility’s normal configuration.

Expected outcome: Monitor settings are restored before return to service.

Electrical Safety Testing

The manual lists patient safety checks and leakage current testing. It specifically lists leakage current measurements using the LCC-1101 leak current checker and states measured values must be recorded.

Protective Earth / Ground Resistance

PM passes if visual inspection passes, the monitor powers on normally, diagnostic / manual check access is successful, the alarm indicator functions correctly, electrical safety checks pass using manual-listed or facility-approved limits, temperature accuracy is within listed ranges, connector-off and sensor-off detection messages appear correctly, and no unresolved technical alarms or unsafe conditions remain.

Remove From Service

Remove from service if physical damage, liquid intrusion, damaged power cord, failed grounding, failed leakage current, failed alarm indicator, failed temperature accuracy, failed disconnect detection, or unresolved fault is found. Remove from service if the monitor cannot complete required checks or cannot be verified safe for patient use.
